The Home Health Quality Improvement (HHQI) National Campaign is a grassroots movement designed to unite home health stakeholders, like Senior Home Care, under the shared vision of reducing avoidable hospitalizations, improving medication management and preventing falls in the home.

In partnership with the HHQI National Campaign initiative, Senior Home Care is a member of the Local Area Networks for Excellence (LANE). As a member, Senior Home Care is a key campaign partner and serves as the central hub of activity in Louisiana and Florida amongst the campaign members and affiliates. LANEs are comprised of home health state associations, Quality Improvement Organizations (QIOs) and other leading home health organizations.

Senior Home Care has begun implementing best practices to decrease acute care hospitalizations and improve patient management of oral medications.
For the remainder of 2010, the company will be implementing best practices for Fall Prevention and Cross Setting Care. HHQI will feature Senior Home Care’s Best Practices for Fall Prevention in the upcoming Best Practices Intervention Package. For more information about the campaign, go to www.homehealthquality.org.
