Once your patient is referred to Senior Home Care or Synergy Home Care, our professional Account Managers and Patient Care Coordinators stay in touch around the clock and gently guide the patient through the intake process, transitioning them to home care with ease.

Senior Home Care utilizes the latest technology -- like Homecare Homebase™, Provider Link Web Portal and ReferralCentral® -- to ensure that our referral sources receive efficient response times and constant access to real-time patient information.
